The Outreach Tracker is where patient handoff happens.
Here’s how that flow works:
Codent conducts outreach and handles patient questions.
Once a patient is ready to move forward, they are clearly marked in the Outreach Tracker.
At that point, the patient is ready for your team to:
Confirm appointment details
Schedule treatment
Handle any final logistics specific to your office
Codent’s responsibility: Outreach, follow-up, education, and identifying patients ready to proceed
Practice responsibility: Scheduling and completing care
Timely follow-up here is critical — the longer patients sit without being scheduled, the more momentum can be lost. Our goal is to make this handoff clear and seamless so patients move quickly from “yes” to scheduled care.
